Make sure you bring your physical therapy referral (provided to you by your doctor) and your payment information. If your insurance is covering the cost of physical therapy, please bring your insurance card(s). If you are covered by Workers' Compensation, bring your letter of authorization, which should include your claim number, your case manager's contact information, and the number of visits that are authorized. If you are covered by first party auto insurance, make sure you bring this information.
