These are the steps you must take in order to attend physical therapy:

1) You will need an evaluation by a physician who is a provider for workers compensation

2) Obtain a referral for physical therapy

3) Most importantly, you will need an AUTHORIZATION from your workers compensation insurance prior to attending treatment
